Emergency Prescription Service:

Provides short-term assistance for persons who need prescription drugs but have no funds to purchase them. People in need are referred to us by many social service agencies in the community.

Requirements:

  • Must be uninsured
  • Must have a photo id
  • Must have a valid prescription

The Emergency Prescription Service of Kumler Neighborhood Ministries cannot fill prescriptions from Capitol Community or Mental Health.

Indigent Pharmaceutical Program:

Assists persons who need prescription medications to manage long-term, life threatening medical problems. A case manager helps access programs which provide long-term medications for the indigent. These programs are offered by manufacturers of prescription medications but require the cooperation and coordination of patient, physician and vendor. This program has received grants from state, local and federal sources. Since people are usually out of their medication when they come to us, we can usually purchase two months worth of medication for them until their free medicine program benefits take effect.

To Qualify Applicants Must Provide the Following Items:

1. Proof of I.D. (one photo)

  • Driver license or State ID

2. Proof of Income:

  • Social Security award paper
  • a copy of bank record if direct deposit
  • check copy or stub (last four checks received)
  • checking and savings account statements
  • tax return
  • IF ON CIRCUIT BREAKER: A copy of applicaiton with acceptance or denial letter
  • If receiving child support proof of

3. Medications:

  • Medications bottles (if new prescriptions bring in scripts)
  • Any outstanding medical bills (Monthly amount paid)

4. Proof of rent or housing

  • If owns own home must provide real estate tax bill
  • If renting, a copy of a rent receipt or canceled check of rent payment

5. Proof of utilities

  • telephone bill
  • cable bill
  • gas bill
  • electric bill

6. Proof of dependents

  • marriage certificate
  • child's birth certificate or public aid card
  • Social Security Cards for all family members

7. Name, address, and phone number of Doctor or Doctors with the correct spelling

8. If the client has a social worker we need a name and phone number of that case worker
